The Criteria for waivers based on research remains unchanged, though the application form (HHS 426) has minor revisions. Please follow the instructions on the supplementary information sheet. Please note that the National Institutes of Health (NIH), Centers for Disease Control and Prevention (CDC), and the Food and Drug Administration (FDA) are all components of the Department of Health and Human Services. All applications for waiver of the J-1 Visa relating to research, must be reviewed by the Department's Exchange Visitor Waiver Review Board.
