DWSRF Management Manual

The DWSRF program requires that states submit specific documentation in order to receive a capitalization grant.  EPA has developed a series of illustrative examples of this material for the fictitious State of New Water.  The materials are not intended to be prescriptive, but are meant to serve as examples for states and the public of the types of documents a state must submit in the program.   The files are provided in an Acrobat PDF and WordPerfect 8 format. 
 

SF-424 This is the standard assistance form a state must submit to EPA in order to receive a grant.  (WP8)
OA Some of the information a state must submit to EPA in order to receive a grant does not change annually.  At its option a state may include this information in an Operating Agreement between the state agency and EPA.   (WP8)
IUP Each year a state must prepare an Intended Use Plan describing how it will direct funds within the program.  This plan must be made available to the public for review and comment.  It must include a description of the criteria the state uses to prioritize projects for funding, an identification of the projects to be funded, and a description of how it will use set-aside funds.  (WP8)
BR Every two years a state must prepare a Biennial Report  describing the status of its DWSRF program.  Some states elect to submit an annual report.  Among the required elements, a state must describe how it has addressed the goals and objectives outlined in its IUP, show which projects received funds, and describe how set-aside funds were used.  (WP8)
Audit States must provide annual audited financial statements for the DWSRF program.  Examples are provided for two years -  FY1999 and FY2000. (Acrobat only)
