Current status


The Center for Health Statistics is the state's depository for all vital records and is the major information source for vital statistics and health survey data about Oregonians. CHS publishes Annual Reports on natality (Vol 1), mortality (Vol 2) and a County Data Book with vital statistics data by county.

Ordering Vital Records: Go to Vital Records Frequently Asked Questions (FAQ) to find out about procedures and costs for requesting vital records, or Pre-adoption Birth Records (FAQs) has information on the implementation of Ballot Measure 58 (access to pre-adoption birth records and filing of birth parent contact preference form).

Vital Records fees are used to support the services provided by state and county vital records offices.
