RECOMMENDING OFFICE : Baltimore District (HFR-MA200) : DIOP, HFC-170 REASON FOR ALERT : On September 6, 1991, Baltimore District was informed by the Maryland State Department of Health of an outbreak of Cholera. Three individuals became ill and were found to be infected with Cholera. The strain was confirmed by CDC as Vibrio Cholerae, 01, Ogawa, El Tor, Toxigenic N. The product implicated in this outbreak was a dessert made with coconut milk. A number of different products were collected by Baltimore district as a result of a follow-up investigation; laboratory analysis of a sample of fresh frozen coconut milk identified the presence of V. Cholerae , 01, Ogawa, Toxigenic, which was confirmed by CFSAN. The product was also found to contain Salmonella. The product contaminated with V. Cholerae is Asian Best Brand of frozen fresh coconut milk packed in 8 oz. plastic bags. The manufacturer is identified as CHAMP GROUP ENTERPRISE CO., LTD., Bangkok, Thailand. The product was distributed in Asian specialty markets along the east coast and in the midwest by ^ ^. Other shippers and/or distributors have not been identified. A separate sample of a different product, from the same manufacturer, coconut candy was found to contain Salmonella. As a result of this import alert's intensified coverage of coconut products, it has become necessary to expand the attachments for products and manufacturers from other countries which have been found to be violative. IMPORT ALERT #23-12 REVISED 4/14/94 PAGE 3 INSTRUCTIONS: Automatically detain all shipments of fresh, frozen, and dried coconut products manufactured by manufacturers/shippers listed in the attachments, using the appropriate charges as noted. In addition, intensified coverage of fresh and frozen products containing coconut milk or coconut meat from all other manufacturers/shippers in all countries is indicated for the presence of Vibrio Cholerae, Salmonella, and E. coli as per the following instructions: A sample will consist of 10 - 227 gm (8 oz) subsamples. Test for V. Cholerae using the BAM, Chapter 9 Vibrio method, dated August 30, 1991; Salmonella and E. coli. If the level is found to be equal to or greater than 10,000/gm then perform ETEC. * Districts should also review the V. cholerae procedures outlined in the Domestic and Imported Foods Assignment dated 3/1/93 for additional guidance. DO NOT COLLECT SAMPLES OF CANNED THERMALLY PROCESSED PRODUCTS. For all samples analyzed submit a copy of Form FDA 465 (Analytical Summary Sheet) on an as-completed basis to Field Programs Branch, CFSAN, HFF-26. Report presumptive positive Cholerae findings to Dr. Joseph Madden at FTS (202) 205-4217 or Dr. Elisa Elliot, FTS (202) 205-4192. Report all non-01 and 01 positive confirmed cholerae findings to CFSAN/Division of Enforcement, Import Branch, HFS-606, Len Valenti, at FTS/commercial * (202)205-4726 or at FAX *(202)260-0208. All presumptive positive samples must be reported to DFS by FAX FTS (301)443-6388.
