Military Academy Nominations

Thank you for your interest in the U.S. Service Academies.  As your 4th District Representative in the U.S. House of Representatives, each year I have the privilege of nominating a limited number of students to four of the five service academies.

United States Service Academy Application Checklist:

1. Candidate Application – Print and complete the application packet and attach an appropriate photograph. Formal candidacy is established upon receipt of your completed application packet by the Decatur office. Please note that you are also required to complete an application for each academy and/or university to which you are seeking admittance.
2. Letters of Recommendation – Each applicant is required to submit two (2) signed letters of recommendation. This form can be completed by your Principal, Guidance Counselor, Math Teacher, English Teacher, Science Teacher, and/or one adult non-family member. Letters referring to the candidate’s character and leadership qualities will be beneficial during the selection process. Each recommender may attach a separate letter to the signed forms. Each form should be completed and submitted in a sealed envelope with the recommender's signature across the seal. All letters must include a return address and phone number.
3. Academic Transcript – Official transcripts must be in a sealed envelope with a signature across the seal. If you have already graduated from high school, please include information outlining your academic and/or employment activities since graduation.
4. Standardizes Test Results – ACT and SAT scores must be submitted with your application packet. Please note that it can take up to six weeks to receive your test scores so plan testing accordingly. Scores printed on your transcripts are not acceptable. If you would like to send directly from the testing agencies, our codes are: ACT | 7354 SAT | 1530(If you are taking your SAT or ACT in October, printed scores will be accepted at the interview.)

Please submit the application printed in portrait layout, signed by hand in all required spaces.

†Please Note: you do not have to reside in the fourth district to apply for the U.S. Merchant Marine Academy with our office. You may apply with other offices for the Merchant Marine Academy nomination.
