Charitable Organization Initial Registration Form Most charitable organizations are required by Missouri statute to register before soliciting for donations in Missouri. PDF
Charitable Organization Annual Report Form Many of these charitable organizations also are required to file annual reports and, on request, disclose how much of the solicited money is spent on fundraising costs. Charities exempt from this provision include religious organizations and educational institutions. PDF

For Professional Fundraisers
Professional Fundraiser Initial Registration Form All professional fundraisers must register with the Missouri Attorney General before they solicit charitable donations in Missouri for any charitable organization.    PDF
Professional Fundraiser Renewal Application Registration is effective for a one year period from the date of issuance. All professional fundraisers must file a renewal application before the end of that one year period.    PDF
Professional Fundraiser Reinstatement Form All professional fundraisers who fail to file a renewal application with the Missouri Attorney General within 45 days after the renewal date shall forfeit its registration and need to fill out this form and submit it along with the required fee. PDF
Solicitor Registration Statement All professional fundraisers shall return this completed statement to the Missouri Attorney General within 15 days after the hire of each solicitor. PDF

Notice of Dissolution A not-for-profit corporation planning to dissolve must give notice of the dissolution at or before the time it delivers articles of dissolution to the Secretary of State's Office.  PDF 
Going-out-of-Business Sales
Registration Affidavit Businesses conducting going-out-of-business sales must register the sale with the Attorney General's Office at least 10 days before it begins. PDF
Extension Affidavit Going-out-of-business sales should last no more than 60 days. If a sale must exceed that length, an extension affidavit must be filed. PDF
Health Spas
Health Spa Initial Registration Statement Health spas new to Missouri should complete the Health Spa Initial Registration Statement. PDF
Annual Renewal Registration Statement Existing health spas should complete the Health Spa Annual Renewal Registration Statement. PDF
Health Spa Reinstatement Form  All Health Spas who fail to file a renewal registration statement within 45 days after the renewal date shall forfeit its registration and need to fill out this form and submit it along with the required fee. PDF
Health Spa Irrevocable Letter of Credit Health spas new to Missouri should also file and maintain with the office of the attorney general a bond with corporate surety or a letter of credit from a bank insured by the FDIC. PDF
Travel Clubs
Travel Club Registration Statement Previously unregistered travel clubs should complete the Travel Club Registration Statement. PDF
Travel Club Renewal Statement Previously registered travel clubs should complete the Travel Club Renewal Statement. PDF
Travel Club Irrevocable Letter of Credit Each Travel Club registration shall be accompanied by a certificate of deposit or a letter of credit that is issued by a banking institution with assets of at least seventy-five million dollars. The certificate of deposit or letter of credit shall be in the amount of at least two hundred fifty thousand dollars. The Account Ownership of the certificate of deposit will be a multi-party account naming the Missouri Attorney General’s Office and the travel club as account ownership. PDF
