PASSPORTS

The American Consulate Naha provides a wide range of passport services to Americans residing in Okinawa and the Amami Oshima Island group, parts of Kagoshima Prefecture. For most adults, many of these services are available without the need to come to our office. 

The rules on how to apply for a U.S. passport have changed, requiring more people to apply in person and allowing fewer people to apply by mail. Under the new rules, only adults who were issued a ten year passport on or after their 16th birthday may apply for a renewal passport by mail. All others must appear in person. This change is effective worldwide immediately and is not specific to Japan. 

All walk-in application fees are payable in cash (dollars or yen) or by money order made payable to "US Embassy, Tokyo". All Military Personnel and their dependents application fees are payable by money order only in the exact US$ amount made payable to "US Embassy, Tokyo". Processing time is about two-three weeks after we receive your application in good order, as all passport printing now must be done in the U.S. However, because computers break, typhoons happen and mail is sometimes delivered late, please don't make any unchangeable travel plans until you have your new passport in hand. MAROON OFFICIAL and BLACK DIPLOMATIC passport applications takes 2-3 months. Please apply well in advance. All applicants 14 or older must sign their own application. A parent must sign on behalf of a child younger than 14 years of age. On July 2, 2001, the Department of State implemented a new law regarding the passport applications of minor U.S. citizens under age 14. Under this new law, a person applying for a U.S. passport for a child under age 14 must demonstrate that both parents consent to the issuance of a passport to the child or that the applying parent has sole authority to obtain the passport. Passport applications made at domestic U.S. passport agencies in the U.S. and at U.S. consular offices abroad are both covered by this law. All applicants must be in Japan for us to process their application. The Consulate is open Monday - Friday, 8:30 - 11:30 AM. The Consulate is closed on Japanese and American holidays.

ALL US MILITARY PERSONNEL AND THEIR DEPENDENTS MUST OBTAIN APPLICATIONS FROM AND APPLY THROUGH DESIGNATED MILITARY CHANNELS IN ORDER TO RECEIVE PASSPORT SERVICES. US MILITARY PERSONNEL AND THEIR DEPENDENTS CANNOT APPLY IN PERSON AT THE CONSULATE UNLESS THEY HAVE RECEIVED ADVANCE AUTHORIZATION FROM THE CONSULAR OFFICER TO DO SO.

If a passport has been lost or stolen, a police report documenting the loss or theft must also be presented. Passports for adults are generally valid for 10 years; children's passports for only 5 years. There is no charge for amending or adding pages to a passport.

Note that whether you apply by mail, or in person, for a renewal passport, we always cancel and return your old passport to you. This makes it easier to renew your Japanese visa, as well as giving you the old passport to help you remember your travels

RENEWING A PASSPORT BY MAIL

You can renew your existing passport by mail without a personal appearance only if-- 

You were issued your most recent U.S. passport at age 16 or older and within the past fifteen years, and 

You can submit your most recent passport (even if it has already expired within the past fifteen years as long as you meet the other criteria, below), and 

Your most recent U.S. passport was issued for full validity and 

Your name will remain the same as on your most recent passport. 

If you are not eligible to apply for your passport by mail, you must appear at the Consulate in person and execute a passport application. Please refer to "Applying for a Passport in Person".

Application Form

Please complete one copy of Form DSP-82 , available for download (forms are also available for pick-up in our office and are available on base for military persons). 

Two Photos

The photographs must be identical and should show your full face taken against a light background. Either black and white or color photos are acceptable. Photos must be 5 centimeters square and no larger than 3.5 centimeters from the chin to the top of the head including hair.

Old Passport

You must submit your old passport with your mail-in application. We will cancel the old passport and return it to you along with the new one. 

Self-Addressed, Stamped Envelope

Please include a self-addressed, stamped envelope. Postage is Y270. 

Fee

The fee for passport renewal by mail is $55. 

All mail-in applications must be paid in dollars by International Postal Money Order (kokusai yubin kawase) in US dollars made out to "US Embassy Tokyo." This money order is available from the Japanese Post Office. 

We cannot accept cash through the mail.

APPLYING FOR A PASSPORT IN PERSON

The information on this page applies to: 1) Minors age 14 and over, 2) Adults applying for their first passport, 3) Persons who cannot produce a full validity passport issued after age 16, and any others who need to apply in person.

Our hours are Monday through Friday, 8:30-11:30 AM. We are closed on Saturdays, Sundays and on American and Japanese holidays.

Personal Appearance

All persons who cannot produce a full validity passport issued at age 16 or older must apply in person. Minors age 14 and over must appear in person but do not need to be accompanied by a parent. 

Proof of Your Identity

ID can be proved through a government-issued photo ID document. These include but are not limited to a driver's license, Japanese Alien Registration card, U.S. state ID, expired passport and/or Military ID. All documents must be originals. 

Proof of Your Citizenship

is proven through an official birth certificate or naturalization/citizenship certificate, or a Consular Report of Birth. All documents must be originals. Military orders and/or military ID are not acceptable proof of American Citizenship.

RENEWING A CHILD'S PASSPORT (Under Age 14)

Personal Appearance

All children under age 14 must apply in person, accompanied by at least one parent. They cannot apply by mail; no exceptions can be made unfortunately. 

ALL US MILITARY DEPENDENTS MUST OBTAIN APPLICATIONS FROM AND APPLY THROUGH DESIGNATED MILITARY CHANNELS IN ORDER TO RECEIVE PASSPORT SERVICES. US MILITARY DEPENDENTS CANNOT APPLY IN PERSON AT THE CONSULATE UNLESS THEY HAVE RECEIVED ADVANCE AUTHORIZATION FROM THE CONSULAR OFFICER TO DO SO.

Two-Parent Consent Required

The two-parent passport consent requirement for minors under age 14 applies to all passport services. To comply with this two-parent passport consent statute, you must: 


Submit the other parent's written consent to the application using this form. The form need not be notarized. You must also include the child's original birth certificate as evidence of the two parents' names and bring in with you the other parent's original passport. 

If only one parent signs the application with no consent from the other parent, the signing parent must establish sole authority to execute the application through written proof of sole custody, for example, following a divorce. 

Or, both parents may appear in person with their passports and sign for children under age 14. You must also bring the child's original birth certificate as evidence of the two parents' names. 

Here are the things you'll need to renew your child's passport. 

Application Form

Both parents sign their names and the child's name for applicants under age 14. The child does not sign for him/herself. All applicants must be in Japan for us to process their application.

Adoption decree (with adopting parents' names); or

Court order establishing custody; or 

Court order establishing guardianship. 

If the parent(s)/guardian current name(s) is/are other than what is stated on these documents, evidence of legal name change (e.g. marriage, divorce, court-ordered name change) is required. 

Fee

The fee for a renewal passport is $70 for children. We accept cash, US$ or Japanese Yen. We cannot accept checks.

REPLACING A LOST/STOLEN/MUTILATED PASSPORT

The loss or theft of a U.S. passport is a serious matter. Please report the loss to the police. To replace the passport, you must appear in person at the Consulate. 

ALL US MILITARY PERSONNEL AND THEIR DEPENDENTS MUST OBTAIN APPLICATIONS FROM AND APPLY THROUGH DESIGNATED MILITARY CHANNELS IN ORDER TO RECEIVE PASSPORT SERVICES. US MILITARY PERSONNEL AND THEIR DEPENDENTS CANNOT APPLY IN PERSON AT THE CONSULATE UNLESS THEY HAVE RECEIVED ADVANCE AUTHORIZATION FROM THE CONSULAR OFFICER TO DO SO.

Here's What You'll Need

To replace a lost or stolen passport, you must present evidence of your identity and your citizenship. You will also need a police report, fee, photo and an application form.

Proof of Your Identity

is proven through an official birth certificate or naturalization/citizenship certificate, a Consular Report of Birth and/or an expired passport. All documents must be originals. Social Security cards, driver's licenses, Military IDs and/or orders are not proof of U.S. Citizenship. 

Police Report

To replace a lost/stolen passport, also submit a police report documenting the circumstances surrounding the loss or theft. If the police will not issue a report, they will usually issue a report receipt, or a report number. Please bring this in to us with your application materials in lieu of the actual report. 

Affidavit

Please also submit a detailed affidavit of the circumstances surrounding the loss or theft. Here is the form to use, available for download: DSP-64. Please don't sign the affidavit-- you will need to sign it at the Consulate when you apply in person for a new passport. You will need to use the latest version of free Acrobat software to view and print this form. You can download the software.

Minors

Applicants under the age of 14 must meet the requirements imposed by the Law on Passport Applications for Minors.

Passport Applications for Minors

A person applying for a U.S. passport on behalf of a child under age 14 must demonstrate that both parents' consent to the issuance of a passport to the child or that the applying parent has sole authority to obtain the passport.

All passports processed at domestic U.S. passport agencies and at U.S. consular offices abroad are covered by this law. Exceptions to this requirement may be considered on a limited, case-by-case basis in exigent circumstances that require immediate travel.

Congress' purpose in adding this requirement that both parents' consent be demonstrated is to lessen the possibility that a U.S. passport might be used in the course of an international parental child abduction.

Parents having concerns about passport applications for any minor child who may become the victim of an international parental child abduction, including children between the ages of 14 and 18 who are not covered by this new law, should contact the Department of State, Bureau of Consular Affairs, Office of Children's Issues at (202) 663-2641.Note that these requirements are in addition to the standard passport requirements for children.

All documents must be originals. Any non-English document must include a translation. We may request whatever additional documentation is necessary from you to meet these requirements.

(1) You Must Present - Evidence of Child's Relationship to Parents/Guardians

Certified U.S. birth certificate (with parents' names); or

Certified foreign birth certificate (with parents' names and translation); or 

Consular Report of Birth Abroad (FS-240) (with parents' names); or 

Certification of Birth Abroad (DS-1350)(with parents' names); or 

Adoption decree (with adopting parents' names); or 

Court order establishing custody; or 

Court order establishing guardianship. 

If the parent(s)/guardian current name(s) is/are other than what is stated on these documents, evidence of legal name change (e.g. marriage, divorce, court-ordered name change) is required.

(2) You Must Also Present - Parents' Photo Identification

Valid drivers license; or 

Valid military ID; or 

Valid govt ID; or 

Valid U.S. or foreign passport with photo; or 

Certificate of Naturalization or Citizenship with recognizable photo; or 

Alien Registration Card.

(3) You Must Also Present - Parental Consent to Issuance

Both parents sign the child's passport application; or 

One parent signs and submits second parent's signed consent (Download the form; the form does not need to be notarized) authorizing passport issuance for the child; or 

One parent signs and submits primary evidence of sole authority to apply.

Acceptable evidence may include the child's certified U.S. or foreign birth certificate (with translation) listing only the applying parent, a Consular Report of Birth Abroad (FS-240) or Certification of Birth Abroad (DS-1350) listing only the applying parent, a Court order granting sole custody to the applying parent (unless child's travel is restricted by that order), an Adoption decree (if applying parent is sole adopting parent), a court order specifically permitting applying parent's or guardian's travel with the child, a judicial declaration of incompetence of non-applying parent or a death certificate of non-applying parent.

Exceptional Cases

If none of the above documents are available, the applying parent/guardian must apply in person with the child present at the Consulate at the same time.

The applying parent/guardian must submit a signed explanation and documentation showing why the non-applying parent/guardian's consent cannot be obtained. 

Exceptions may be granted only in exigent (e.g. the health or welfare of the child is in jeopardy) or special family (e.g. the non-signing parent's whereabouts are unknown, he/she is medically incapable or is incarcerated) circumstances. 

A third-party acting in loco parents on behalf of child under age 14 must submit a notarized consent from both parents or guardians authorizing this third party to apply for the child's passport. When this notarized consent is from only one parent/guardian, the third party must present evidence of the authorizing parent/guardian's sole custody. 

Note that if you do not provide your Social Security Number, the Internal Revenue Service may impose a $500 penalty. 

CHANGING YOUR NAME IN YOUR PASSPORT

Your passport may be amended to show your married name or a legal change of name. Processing time is one week.

Documentation

You'll need to show us legal documentation supporting any requested change. For example, your original marriage certificate showing your new spouse's name, an original court order showing a legal name change or an original divorce decree stipulating the restoration of a maiden name. We cannot accept copies, and we reserve the right to request additional documentation.

Additional Pages - By Mail

If your full-validity passport has not yet expired, but has no blank pages left, we can add additional pages for you free of charge by mail. Processing time is one week.

Two-Parent Consent Required for Adding Pages 

 

The two-parent passport consent requirement for minors under age 14 applies to all passport services including requests for extra passport supplement pages.

 

To comply with this two-parent passport consent statute, you must:

Submit the other parent's written consent to the application using this form (when applying by mail or in person). The form need not be notarized. You must also include the child's birth certificate as evidence of the two parents' names.

REGISTRATION

We recommend that anyone who will reside in Japan for more than three months register with the nearest U.S. consular office. Registration confirms your identity as a U.S. citizen and provides us with contact information in the event of an emergency. 

Registering with the Consulate (Online or Fax)

Registration requires proof of U.S. citizenship and a local address, and if possible a telephone number, email address and emergency contact information (parents for minor age children). Proof of U.S. citizenship includes a full validity passport or U.S. birth certificate, or certified copies of either. The documents may be brought to the Consulate in person, mailed to the Consulate at (American mailing address) PSC 556, Box 840, FPO AP 96386, or (Japanese mailing address) 2564 Nishihara, Urasoe City, Okinawa 901-0121 (Japan), or faxed to the Consulate at (098) 876-4243.

REPORT OF BIRTH

Congratulations on your child's birth! All U.S. citizens born overseas are documented with a "Consular Report of Birth Abroad", which functions both as an English-language birth document and as primary evidence of the child's U.S. citizenship.

To transmit US citizenship to a child born overseas, you must be a US citizen and have been in the US for five years (including two years after your 14th birthday), meeting all requirements prior to your child's birth. For two US citizen parents to transmit US citizenship to their child, one parent must have resided in the US for any length of time prior to the child's birth. Adjudication of the application is based on our review of the following documents and parents' statements. We may request other information at any time. Processing time is normally three weeks. Please apply in person at the Consulate General Monday-Friday, 8:30 - 11:00 AM. The Consulate is closed on Saturdays, Sundays and both American and Japanese holidays.

SOFA status personnel:

All SOFA status personnel must apply for their child’s Consular Report of Birth and passport through the Birth Registration Office located at the Naval Hospital, Camp Lester. The office has all the necessary application forms for the birth registration and passport process. Please contact them directly for further information, tel: 643-7516. The U.S. citizen parent must execute the application for the Consular Report of Birth Abroad. Both parents must appear in person at the birth registration office in order to apply for the child’s passport. The child must be present.

Non-SOFA U.S. citizens:

All non-SOFA status U.S. citizens must apply for a Consular Report of Birth at the nearest American consular office. The American Consulate General, Naha, has all the necessary application forms for the birth registration and passport process. The U.S. citizen parent must execute the application for the Consular Report of Birth Abroad. Both parents must appear in person before the consular officer to apply for the child’s passport. The child must be present.

  • PARENTS' PROOF OF US CITIZENSHIP: US passport, naturalization certificate or original/certified true copy of birth certificate from a US state or territory.
  • PROOF OF CHILD' BIRTH: official document containing child's and parents' names, date and place of child's birth and signature of attending physician/official records custodian AND, for all non-USFJ births, a shusshou todoke kisaijiko shomeisho (certificate of acceptance of birth report) or koseki tohon (family register).
  • PROOF OF PARENTS' MARRIAGE (if applicable): original/certified true copy of marriage certificate; if the marriage occurred outside the US, the original marriage certificate and a translation are required.
  • PROOF OF TERMINATION OF ALL PRIOR MARRIAGES OF PARENTS: original/certified true copies of divorce decrees or original death certificate.
  • ONE PHOTOCOPY of each document listed in items 1, 2, 3 and 4. We will review the original document and certify the photocopy for our records. The original will be returned to you.
  • APPLICATION FOR A CONSULAR REPORT OF BIRTH (FS-579 or DS-2029) You can download form DS-2029 (formerly known as FS-579) to print out and complete by hand. You can also download a compressed version of this form in MS Word format to complete on-screen and print out.
  • AFFIDAVIT OF PARENTAGE: required if the child was conceived out-of-wedlock or if no proof is presented of parents' marriage or termination of any prior marriages.
  • US PASSPORT APPLICATION (DSP-11, brown color): we strongly suggest that you obtain a passport for your child at the same time you obtain a consular report of birth. All US citizens, including children, must bear their own US passport to enter the US
  • Passport Application (DSP-11): Please submit one copy of Form DSP-11, available for download (forms are also available for pick-up in our office). Though the instructions for world-wide use say only 8.5" x 11" paper is acceptable, we can accept A4 sized copies if you print it out at home.

Two Parent Consent for Passport Issuance 

All applicants under the age of 14 must meet the requirements listed in the Law on Passport Applications for Minors. In most instances this means that both parents must sign your child's passport application, even if one parent is not an American.

Parents can consent in one of two ways:

Both parents can come to the Consulate with their passports and sign together, in person, or...

One parent may sign in person and the other parent give his/her consent through a written affidavit (One parent signs and submits second parent's signed consent; download the affidavit form. The form does not need to be notarized). Follow this link to learn more about these requirements. Note the parent who signs in person will need to still have the non-present parent's passport with him/her.

  • TWO IDENTICAL PASSPORT PHOTOS: non-digitized, 2x2 inches, color or black and white, on white background showing full face and a good likeness of applicant.
  • EVIDENCE OF US CITIZEN PARENT'S PHYSICAL PRESENCE IN US: work, social security, tax or school records may be needed when parent's physical presence in the US is unclear.
  • EVIDENCE OF PARENTS' PHYSICAL PRESENCE TOGETHER AT THE TIME OF CONCEPTION: required if the child was conceived out of wedlock, e.g. passports, travel orders, leases, etc...
  • FEES: US$65 for the Consular Report of Birth Abroad, US$70 for the passport. Fees are payable in cash by dollars or yen or by money order made payable to "US Embassy, Tokyo". Personal checks are not accepted. 
  • SOCIAL SECURITY CARD APPLICATION: may be submitted concurrently with the Consular Report of Birth application. Allow up to 6 months to receive a Social Security card.
  • COMPLETE ENGLISH TRANSLATION OF ANY NON-ENGLISH LANGUAGE DOCUMENT

All birth, adoption, marriage, death or divorce certificates must bear the ORIGINAL seal, stamp or signature of the OFFICIAL CUSTODIAN of the document. Notarized copies, church records and uncertified photocopies are not official documents and may be invalid as primary evidence.

OFFICIAL DUPLICATE COPIES of the Report of Birth (DS-1350) are available only from the Department of State in Washington, D.C. at

No records are kept at the American Consulate General Naha.
