The loss or theft of a U.S. passport is a serious matter. Please report the loss to the police. To replace the passport, you must appear in person at the Consulate.
ALL US MILITARY PERSONNEL AND THEIR DEPENDENTS MUST OBTAIN APPLICATIONS FROM AND APPLY THROUGH DESIGNATED MILITARY CHANNELS IN ORDER TO RECEIVE PASSPORT SERVICES. US MILITARY PERSONNEL AND THEIR DEPENDENTS CANNOT APPLY IN PERSON AT THE CONSULATE UNLESS THEY HAVE RECEIVED ADVANCE AUTHORIZATION FROM THE CONSULAR OFFICER TO DO SO.
The Consulate is open Monday - Friday, 8:30 - 11:30 AM. The Consulate is closed on Japanese and American holidays.
Here's What You'll Need
To replace a lost or stolen passport, you must present evidence of your identity and your citizenship. You will also need a police report, fee, photo and an application form.
Proof of Your Identity
ID can be proved through a government-issued photo ID document. These include but are not limited to a driver's license, Japanese Alien Registration card, U.S. state ID, expired passport and/or Military ID. All documents must be originals.
Proof of Your Citizenship
is proven through an official birth certificate or naturalization/citizenship certificate, a Consular Report of Birth and/or an expired passport. All documents must be originals. Social Security cards, driver's licenses, Military IDs and/or orders are not proof of U.S. Citizenship.
Police Report
To replace a lost/stolen passport, also submit a police report documenting the circumstances surrounding the loss or theft. If the police will not issue a report, they will usually issue a report receipt, or a report number. Please bring this in to us with your application materials in lieu of the actual report.

Please also submit a detailed affidavit of the circumstances surrounding the loss or theft. Here is the form to use, available for download:

Minors

Two Photos
The photographs must be identical and should show your full face taken against a light background. Either black and white or color photos are acceptable. Photos must be 5 centimeters square and no larger than 3.5 centimeters from the chin to the top of the head including hair.
Fee
The fee to replace a lost or stolen passport is $85 for adults and $70 under those age 16 or younger. We accept only cash, US$ or Japanese Yen. We cannot accept personal checks.
Passport Applications for Minors
A person applying for a U.S. passport on behalf of a child under age 14 must demonstrate that both parents' consent to the issuance of a passport to the child or that the applying parent has sole authority to obtain the
passport.
All passports processed at domestic U.S. passport agencies and at U.S. consular offices abroad are covered by this law. Exceptions to this requirement may be considered on a limited, case-by-case basis in exigent circumstances that require immediate
travel.
Congress' purpose in adding this requirement that both parents' consent be demonstrated is to lessen the possibility that a U.S. passport might be used in the course of an international parental child
abduction.
Parents having concerns about passport applications for any minor child who may become the victim of an international parental child abduction, including children between the ages of 14 and 18 who are not covered by this new law, should contact the Department of State, Bureau of Consular Affairs, Office of Children's Issues at (202) 663-2641.Note that these requirements are in addition to the standard passport requirements for
children.
All documents must be originals. Any non-English document must include a translation. We may request whatever additional documentation is necessary from you to meet these requirements.
(1) You Must Present - Evidence of Child's Relationship to Parents/Guardians
Certified U.S. birth certificate (with parents' names);
or
Certified foreign birth certificate (with parents' names and translation); or
Consular Report of Birth Abroad (FS-240) (with parents' names); or
Certification of Birth Abroad (DS-1350)(with parents' names); or
Adoption decree (with adopting parents' names); or
Court order establishing custody; or
Court order establishing guardianship.
If the parent(s)/guardian current name(s) is/are other than what is stated on these documents, evidence of legal name change (e.g. marriage, divorce, court-ordered name change) is required.
(2) You Must Also Present - Parents' Photo Identification
Valid drivers license; or
Valid military ID; or
Valid govt ID; or
Valid U.S. or foreign passport with photo; or
Certificate of Naturalization or Citizenship with recognizable photo; or
Alien Registration Card.
(3) You Must Also Present - Parental Consent to Issuance
Both parents sign the child's passport application; or
One parent signs and submits second parent's signed consent (Download the form; the form does not need to be notarized) authorizing passport issuance for the child; or
One parent signs and submits primary evidence of sole authority to apply.
Acceptable evidence may include the child's certified U.S. or foreign birth certificate (with translation) listing only the applying parent, a Consular Report of Birth Abroad (FS-240) or Certification of Birth Abroad (DS-1350) listing only the applying parent, a Court order granting sole custody to the applying parent (unless child's travel is restricted by that order), an Adoption decree (if applying parent is sole adopting parent), a court order specifically permitting applying parent's or guardian's travel with the child, a judicial declaration of incompetence of non-applying parent or a death certificate of non-applying parent.
Exceptional Cases
If none of the above documents are available, the applying parent/guardian must apply in person with the child present at the Consulate at the same time.
The applying parent/guardian must submit a signed explanation and documentation showing why the non-applying parent/guardian's consent cannot be obtained.
Exceptions may be granted only in exigent (e.g. the health or welfare of the child is in jeopardy) or special family (e.g. the non-signing parent's whereabouts are unknown, he/she is medically incapable or is incarcerated) circumstances.
A third-party acting in loco parents on behalf of child under age 14 must submit a notarized consent from both parents or guardians authorizing this third party to apply for the child's passport. When this notarized consent is from only one parent/guardian, the third party must present evidence of the authorizing parent/guardian's sole custody.
Note that if you do not provide your Social Security Number, the Internal Revenue Service may impose a $500 penalty. |
