Change Releasability Status of your Mailing Address
The Aviation Investment and Reform Act for the 21st
Century, in addition to other statutory requirements, requires us to make airmen certificate
information available to the public. Airmen meeting the active criteria are sent a notification of
these requirements, and are given the opportunity to withhold their address information from
release. If your address appears in this data base and you would like to remove it, or if it does
not appear and you would like to add it, you may write or email us your preference. You may provide
us with your preference at any time.
When corresponding with us, please be sure to include your full name, address, date of birth, and
certificate number, so that we may properly identify your record. Send written inquiries to:

Application for Replacement of Lost, Destroyed, or Paper Airman Certificate(s) and Knowledge Test Report(s)
For replacement of lost, destroyed, or paper certificates, submit a signed, written request stating your name,
date and place of birth, social security number and/or certificate number, and the reason for replacement to

Include a check or money order for $2 (U.S. Funds) for each certificate, made payable to the FAA. If
your current address is listed as a Post Office Box (POB), General Delivery, Rural Route, or Star
Route, please provide directions or a map for locating your residence. Allow 4 to 6 weeks for
processing. Only one copy of each certificate can be issued. The original date of issue cannot be
placed on a replacement certificate. Expired certificates cannot be issued; however, an expired CFI
letter can be requested at no charge.
*NOTE: In compliance with the policy dated July 23, 2002, airmen applying for a
replacement certificate issued on the basis of a foreign license under 14 CFR Part 61, Section 61.75
or 14 CFR Part 63, Section 63.42, must complete and submit a Verification of Authenticity of Foreign
License, Rating, and Medical Certification form to the Airmen Certification Branch and appear at a FSDO for positive identification to
make application for a replacement certificate.

A post office box is not acceptable as a residence address. A residence address must be furnished;
however, if you wish a post office box -preferred mailing address, you may furnish both. If your
residence address is listed as General Delivery, Rural Route, or Star Route, you must provide
directions, or a diagram, for locating the residence attested by your signature.
A new certificate is not required when updating your address and will not be issued automatically.
Replacement of Your
Lost or Destroyed Knowledge Test Report
For replacement of lost or destroyed knowledge test report for which the certificate has not been
issued, submit a signed, written request stating your name, date of birth, social security number,

accepting facsimile requests for test results, faxing test scores, or giving test scores over the

Report a Change in
Your Name, Nationality/Citizenship, Gender, or Date of Birth
Name change:
To obtain a new airman certificate that reflects a legal name change, it is necessary that you appear
at an FAA Flight Standards District Office (FSDO) for positive identification. A photocopy of a marriage
license, court order, or other valid legal document, which legally verifies the name change, must be
presented to an FAA Inspector.
Nationality/citizenship change:
To obtain a new airman certificate that reflects a nationality/citizenship change, it is necessary that
you appear at an FAA Flight Standards District Office (FSDO) for positive identification. A naturalization
document or other legal document that verifies the nationality/citizenship change must be presented to an FAA Inspector.
Gender change:
To obtain a new airman certificate that reflects a gender change, it is necessary that you appear at an FAA
Flight Standards District Office (FSDO) for positive identification. One or both of the following documents must be
presented to an FAA Inspector:

A court order issued by a court of the United States or it's territories stating that the applicant has
changed his/her gender, and/or
-
A statement from a physician or clinical psychologist treating the applicant that contains:

Identification of the applicant by name and address, and
-
Verification that the applicant is undergoing treatment that has altered or will alter the
gender.
Date of birth change:
To obtain a new airman certificate that reflects a date of birth change, it is necessary that you appear
at an FAA Flight Standards District Office (FSDO) for positive identification. A copy of your birth
certificate or other legal document that verifies the date of birth change, must be presented to an FAA Inspector.

obtain further information regarding the reissuance of your airman certificate.

Change Your Airman Certificate Number
Effective June 1st, 2002, all original airmen certificates will be issued with a unique certificate
number. The Airmen Certification Branch will no longer be assigning Social Security Numbers (SSN) as
the certificate numbers.
Airman certificates presently using the SSN as a certificate number will continue to be issued with
that number unless the airman requests a unique number.
If you hold an airman certificate that uses your SSN as your certificate number and you would like
our office to change that certificate number to a unique number other than your SSN, or you would
like our organization to remove your SSN completely from our official records, please complete,

pertinent information to the mailing address below or create an account and remove your SSN as your certificate number.

Verification of Authenticity of Foreign License, Rating, and
Medical Certification
Effective July 23, 2002, persons applying for a certificate issued on the basis of a foreign license
under the provisions of 14 CFR Part 61, Section 61.75, special purpose pilot authorizations under
Section 61.77, using a pilot certificate issued under Section 61.75 to apply for a commercial pilot
certificate under Section 61.123 (h), applying for an airline transport pilot certificate issued
under Section 61.153 (d) (3), and applying for a certificate issued on the basis of a foreign
license under the provisions of 14 CFR Part 63, Sections 63.23 and 63.42, must have the validity and
currency of the foreign license and medical certificate or endorsement verified by the Civil
Aviation Authority (CAA) that issued those certificates, before making application for an FAA
certificate or authorization.

Send the completed form with
the preferred documents to the Airmen Certification Branch, AFS-760, PO BOX 25082, Oklahoma City, OK
73125-0082 or fax the form and documents to (405) 954-9922. The pre-application documents cannot be
sent electronically.
A person who is applying for a U.S. certificate/rating on the basis of a foreign license must
apply for that certificate at least 90 days before arriving at the designated FAA FSDO where
the applicant intends to receive the U.S. certificate. This initial application step is the
responsibility of the applicant.
The information submitted to the Airmen Certification Branch by the applicant must include the following information:

The name and date of birth of the applicant.

The address where the person wishes to have the verification of the authenticity letter, issued by the FAA’s Airmen Certification Branch, AFS-760, mailed.
-
The certificate number and ratings on the foreign license.
-
The country of issuance of the person’s foreign license.
-
The location of the FAA Flight Standards District Office where the person
intends to make application for his or her U.S. certificate.
-
A statement that his or her foreign license is not under an order of suspension or revocation.
NOTE: The Airmen Certification Branch would prefer to have a copy of the foreign license and medical
certificate or endorsement included with all requests for verification of authenticity of the foreign license.
When verification is received from the CAA, you will receive written notification that a copy has been
forwarded to the Flight Standards District Office (FSDO) you designated in your request. The verification is
valid for 6 calendar months. You may make application for a U.S. certificate at the designated FSDO during that period.

Applicants:
In addition to the procedures stated under Verification of Authenticity of Foreign license, Rating
and Medical Certification above, airmen from the United Kingdom should note the following additional
steps that are required by the United Kingdom CAA to complete this verification process. Please
visit the UK CAA web site at http://www.caa.co.uk/srg/licensing/whatsnew.asp.
The additional forms are required prior to the UK CAA providing any requested information to the
Airmen Certification Branch, AFS-760.
Please note: The Verification of Authenticity of Foreign license, Rating
and Medical Certification form must be submitted directly to the FAA, Airmen Certification Branch,
AFS-760 not to the UK CAA. The information required by the UK CAA must be submitted directly to the
UK CAA and not to the FAA, Airmen Certification Branch, AFS-760.

SEC. 715. PUBLIC AVAILABILITY OF AIRMEN RECORDS.
Section 44703 is amended--
(1) by redesignating subsections (c) through (f) as subsections (d) through (g), respectively; and
(2) by inserting after subsection (b) the following:
(c) PUBLIC INFORMATION--
(1) IN GENERAL--Subject to paragraph (2) and notwithstanding any other provision of law, the
information contained in the records of contents of any airman certificate issued under this section
that is limited to an airman's name, address, and ratings held shall be made available to the
public after the 120th day following the date of the enactment of the Wendell H. Ford Aviation
Investment and Reform Act for the 21st Century.
(2) OPPORTUNITY TO WITHHOLD INFORMATION--Before making any information concerning an airman
available to the public under paragraph (1), the airman shall be given an opportunity to elect that
the information not be made available to the public.
(3) DEVELOPMENT AND IMPLEMENTATION OF PROGRAM--Not later than 60 days after the date of the
enactment of the Wendell H. Ford Aviation Investment and Reform Act for the 21st Century, the
Administrator shall develop and implement, in cooperation with representatives of the aviation
industry, a one-time written notification to airmen to set forth the implications of making
information concerning an airman available to the public under paragraph (1) and to carry out
paragraph (2). The Administrator shall also provide such written notification to each individual who
becomes an airman after such date of enactment.

Releasable Airmen Database Available for
Download
In accordance with Section 715 of the Aviation Investment and
Reform Act for the 21st Century, the Airmen Certification Branch is releasing a database
containing the addresses and pertinent certificate information for those airmen who have
not requested that such information be withheld

CD-ROM version of this database is no longer available as of 18 January 2001.
