Designation of Beneficiary Forms


You cannot change your Designation of Beneficiaries via the automated systems.  Current regulations require that beneficiary forms be maintained in hard copy form in your Official Personnel Folder (OPF) because of the legal issues surrounding the signature.  However, you may complete the forms online, print them, sign them in the presence of two witnesses (anyone that is not named as a beneficiary - your personal acquaintances can best serve as witnesses should the designation be contested in a court of law) and mail them, with original signatures, as indicated.  If you have attachments to any form, they must be signed and witnessed also.

A designation of beneficiary is a legal document outlinining your desire to have your benefits paid out in a particular way upon your death.   There are four types of benefits for which you can designate a beneficiary:  Life Insurance, Unpaid Compensation, Thrift Savings Plan, and Retirement.

It is not required that you designate a beneficiary for your benefits.  If the order of precedence listed below meets your needs, you don't need to do anything. If you wish to name a person or persons not included below, or in a different order, you will need to complete a form to accomplish this.   It is your decision whether to complete beneficiary forms or not.  If you do complete any beneficiary forms, it is your responsibility to keep them up-to-date and reflecting your wishes.

Note:  For life insurance (FEGLI), if there is qualifying court order or an assignment of benefits on file, they take precedence over any any designation of beneficiary on file and the normal order of precedence above.  For more information, click here.


Beneficiary Forms

Federal Employees' Group Life Insurance Program (FEGLI) - Designates distribution of Basic and Optional Life Insurance you are enrolled in at the time of your death. Standard Form 2823, Designation of Beneficiary

Unpaid Compensation - Designates distribution of unpaid compensation that is payable to you at the time of your death, such as final pay and unused annual leave.

Thrift Savings Plan- Designates distribution of your Thrift Savings Account.
TSP-3, Thrift Savings Plan Designation of Beneficiary

Retirement - Designates a beneficiary for lump-sum retirement benefits only and does not affect the right of any person who qualifies to receive survivor annuity benefits.  If you die and there is no one eligible for a survivor annuity, any money left in your retirement fund account and any annuity due and unpaid on the date of your death will be paid out according to the order of precedence.

NOTE:If you have transferred from CSRS to FERS retirement coverage, any previous CSRS SF-2808 you have on file is invalid as of the date of transfer.  You need to complete a FERS SF-3102 if the order of precedence does not meet your needs.

Carefully read the following for additional important information.

You can make a change in beneficiary at any time, without the knowledge or consent of a previous beneficiary.  (Exception is FEGLI if you have assigned your insurance or there is a valid court order on file with the agency or OPM).  Filing a new form will completely cancel any designation you may have previously filed, so be sure to name everyone you wish to designate as beneficiaries on each new form.  If you are not sure whom you have designated as a beneficiary on a previous form, simply complete a new designation.

You should keep a copy for your records whenever you make a change and review them periodically to make sure they still reflect your wishes.  You should complete a new designation form whenever you have a significant change in your life, such as a marriage, divorce, or death.  A marriage or divorce, or other change in family status does not change a beneficiary election on file.  Until you submit another form to cancel prior designations or to designate a new person(s), the original designation remains in force whether it still reflects your intentions or not.

It is not required to change your beneficiary form(s) to update addresses for designees; however, benefits cannot be paid out properly if the named beneficiary cannot be located.

Please Note:  Your beneficiary forms for TSP and CSRS are not valid until they are received at the addresses indicated above.  If you do not send them directly to the appropriate address, and you die before the form is received by the appropriate office, the designation on the beneficiary form will not be valid.  It is your responsibility to send the forms to the appropriate office for processing.

The designation of beneficiary form for FEGLI, FERS, Unpaid Compensation should be mailed to your Human Resource Service Center.  All other designation forms such as the CSRS and TSP-3 should be mailed directly to the address listed on the form.

To determine your servicing HRSC, please refer to block 48 on your SF-50, Notification of Personnel Action and match it to the corresponding 4-digit number listed in parentheses beside the region name below.
