If you have received notice that PBGC is handling your pension plan and you are not yet receiving benefits from PBGC:
Participant Application for Pension Benefits - PBGC Form 700 Use this form only if the following three conditions apply: 1) you have been notified in writing by PBGC that it has taken over responsibility for your pension plan; 2) you are now eligible (or will be eligible within the next ninety (90) days) to receive benefits under the provisions of your pension plan; and 3) you would like to begin receiving your retirement benefit within the next ninety (90) days.
Beneficiary Application for Pension Benefits - PBGC Form 705 Use this form if you are not (1) the spouse of a participant who died prior to retiring and you are entitled to a Qualified Preretirement Survivor Annuity (QPSA) or (2) an alternate payee with a separate interest under a Qualified Domestic Relations Order.
Beneficiary Application for Pension Benefits - OF - PBGC Form 706 Use this form if either of the following conditions apply: (1) you are the spouse of a participant who died prior to retiring and you are entitled to a Qualified Preretirement Survivor Annuity (QPSA) or (2) you are an alternate payee with a separate interest under a Qualified Domestic Relations Order.
General Information Form - PBGC Form 702 If you were notified that PBGC is handling your pension plan and you were not retired when your plan ended, please use this form to provide PBGC with basic information about your participation in your plan. When you are within ninety (90) days of being eligible for benefits under the provisions of your plan and you wish to apply for pension benefits with PBGC, please use the Application for Pension Benefits (PBGC Form 700).
Power of Attorney - PBGC Form 715 This form should be used by pension plan participants or the beneficiary of a deceased participant to designate someone to represent them on matters pertaining to their benefit. This form may also be used by others, such as an alternate payee under a qualified domestic relations order.
Designation of Beneficiary - PBGC Form 707 Use this form to name or change your beneficiary for either 1) payments that PBGC may owe you at the time of your death OR 2) annuity benefits that are paid to your beneficiary after your death. (Joint-and-survivor benefits will be paid only to your spouse at the time your survivor's benefit starts.)
If PBGC has no record of your participation in a plan that it is handling:
Plan Participation Information - PBGC Form 709 If PBGC has no record of your participation in a pension plan that it is handling, please use this form to provide PBGC with information that will enable PBGC to determine if you are entitled to a benefit.
