The Office of Surface Mining uses several standardized forms to collect information from the public and staff. These forms all have been approved by the Office of Management and Budget (OMB) and are in widespread use. They are provided here in Adobe Acrobat format so they can be printed on a local printer and used for "official" business. Questions about the forms or their use should be directed to John Trelease
