Replacing a Lost/Stolen/Mutilated Passport

All applicants whose passport was lost/stolen/mutilated must apply in person, except for U.S. military personnel who must apply through designated military channels for processing by special courier.

We work on replacement passports only during regular business hours, except in the case of clear, documented, life-or-death emergencies, such as the death of a close family member in the U.S. Otherwise, no weekend or after-hours assistance is available. Normal processing time is about three to four weeks.


The American Embassy and our Consulates are closed on both U.S. and Japanese holidays.

Emergency passports issued abroad are valid for no more than 12 months (and may be issued with much shorter periods of validity), and must be replaced by a full-validity passport printed in the U.S. You must return the limited validity emergency passport to obtain a full validity (5 or 10 years validity) passport. Processing a replacement passport may be longer than normal, if, for example, you have shown a pattern of multiple losses. You should not make any unchangeable travel plans until we are able to hand you your new passport.



Here's What You'll Need
To replace a lost or stolen passport, you must present evidence of your identity and your citizenship. You will also need a police report, fee, photo and an application form.


Proof of Your Identity
ID can be proved through a government-issued photo ID document. These include but are not limited to a drivers license, Japanese Alien Registration card, U.S. state ID, expired passport and/or Military ID. All documents must be originals.


Proof of Your Citizenship
Citizenship is proven through an official birth certificate or naturalization/citizenship certificate, a Consular Report of Birth and/or an expired passport. All documents must be originals. Social Security cards, drivers licenses, Military IDs and/or orders are not proof of U.S. Citizenship.

Police Report
To replace a lost/stolen passport, also submit a police report documenting the circumstances surrounding the loss or theft. If the police will not issue a report, they will usually issue a report receipt, or a report number. Please bring this in to us with your application materials in lieu of the actual report.


You will need to provide a translation of the police report. You may use our fill-in-the-blanks police report translation form. You may do the translation yourself, or have a friend help. There is no requirement to use a professional translator. The translator should sign the form in person at our office.


Affidavit
Please also submit a detailed affidavit of the circumstances surrounding the loss or theft. Here is the form to use, available for download: DSP-64. Please don't sign the affidavit-- you will need to sign it at the Embassy when you apply in person for a new passport.

Minors
Applicants under the age of 14 must meet the requirements imposed by the Law on Passport Applications for Minors.

Please submit one copy of Form DSP-11, available for download (forms are also available for pick-up in our office)

Note that the form you print must be clear and sharp, with black print on white paper. The paper must be 8 1/2 inches by 1l inches. Thermal paper, dye-sublimation paper, special inkjet paper, and other shiny papers are not acceptable.


Two Photos
The photos must be sized 2" X 2" (or 5 cm X 5 cm) with a white background. You can have pictures taken at the Embassy in our photo booth for Y1000, Yen cash only. Our Consulates do not all have photo booths available, but can direct you to a nearby photographer.


Be careful with the size-- "passport photos" you get at a local photographer's shop are likely too small and are intended for Japanese people to use to obtain a Japanese passport.

The photo must be 2 inches by 2 inches with the face size as specified. Your browser may not display the graphic above as the right size.


One of the most common problems with passport applications we receive is that the photos are the wrong size. The photo in your current US passport may be a different size from what is needed for a new passport, as standards have changed from time-to-time.


The best way to make sure your photos are the right size is to download and print out a passport application (see above). The drawing of a proper photo is right on the application form.


Fee
The fee to replace a lost or stolen passport is $85 for adults and $70 under those age 16 or younger.


We accept only cash, US$ or Japanese Yen. We cannot accept checks at any of our offices in Japan.


We cannot accept credit cards, except at the Embassy in Tokyo and at our Consulate in Osaka, where you may pay using your VISA, Mastercard, Discover, Diners Club or American Express card.

USFJ members seeking a "No Fee" passport must submit form DD 1056 in lieu of the fee.


Persons replacing Official or Diplomatic passports must submit form DD 1056 or USG orders in lieu of the fee.
