Medical Foster Home Program

The Extended Care Service, Medical Foster Home Program (MFHP) offers safe, long-term care in the home of an approved Medical Foster Home Caregiver. It is an affordable comprehensive care housing alternative for Veterans.

Regular comprehensive case management visits are made by the Home Base Primary Care Team:

  • Nurse Practitioner
  • Physical Therapist
  • Social Worker
  • Dietician
  • Recreation Therapist

Other team members could include, as needed:

  • Psychologist
  • Psychiatrist
  • Nurse

Home Criteria

  • Must be owned or rented by the caregiver and location of the home must be the caregiver's actual place of residence.
  • Be located, designed, equipped, and maintained to ensure a home-like environment and to provide safe care and supervision for all residents.
  • Must be inspected by a VA multidisciplinary team including a VA Safety Officer.
  • Comply with all state and local licensure requirements and regulations ,including construction, fire, maintenance, and sanitation.
  • Meet the applicable provisions of the most current edition of the National Fire Protection Association (NFPA) Life Safety Code
  • Maximum of 3 residents per home

Caregivers Criteria

This is a long term commitment to the Veteran where he/she may live for many years, often for the remainder of their life and the Medical Foster Home Program is seeking dedicated caregivers, who live in ranch homes, with compassion, skills and a heart desire to care for our disabled Veterans with complex medical needs.

  • Caregivers live in home and are experienced with providing daily hands on patient care
  • Must be physically able to provide the needed care per physician determination
  • Provide a compassionate, friendly, family environment. This is not shift care nursing
  • Home must be owned or rented by the caregiver
  • Must be 21 years or older and financially stable
  • Must have written backup plan if unable to provide care
  • Must pass initial and annual inspection by the VA Inspection Team
  • Must be flexible and have positive problem solving skills
  • Must work with the VA Home Based Primary Care or Spinal Cord Injury Team
  • Must complete annual Caregiver training

Funding

Veteran pays the Medical Foster Home Caregiver based on his/her care needs. The cost of care ranges between $2,000 - $4,000 per month and is based on the care needs of the Veteran.

Veteran Information

  • Furnished private room
  • 24-hour plan of care
  • The caregiver lives in home and provides hands on daily care
  • Medication Management
  • Personal care (dressing, grooming, bathing etc.)
  • Meals, laundry, cleaning
  • Flexible daily routine with various recreational options
  • High levels of caregiver and Veteran satisfaction
  • A signed care agreement between caregiver, Veteran or his/her legal representative
  • Home Based Primary Care provided for the Veteran in the Medical Foster Home
  • Exact locations are dependent upon the Home Based Primary Care catchment area
