Visa Application Forms

To download the non-immigrant visa forms, you will need Adobe Acrobat Reader. If you do not have Adobe Acrobat Reader, you can download it for free here:

All visa applicants are encouraged to use the DS-156 form in English. This form, which is filled out online, creates a special barcode on the application. Consular Section employees will expedite the interview process for applicants who present this form on the day of their scheduled interview. The DS-156 Form in Romanian does not contain a barcode, but can still be used to apply for a nonimmigrant visa.

The DS-156 needs to be completed by all non-immigrant visa applicants. For persons under the age of 16, the form may be completed and signed by one of the parents or a legal guardian:

The following supplemental form (DS-157) needs to be completed by all male non-immigrant visa applicants between the ages of 16-45, regardless of nationality:

The following supplemental form (DS-158) needs to be completed by all F, J, and M category visa applicants, including spouses and children applying for F2, J2, and M2 visas, regardless of nationality.
