Forms to Update Your Pension Account

Have you moved? Do you have a new telephone number? Are there changes in your life (such as marriage) that PBGC needs to know? You can get forms to make updates to your PBGC account. If you have any questions, please contact the Customer Service Center.

How to View PDF Files

If you are currently receiving periodic benefits from PBGC:

  • Application for Electronic Direct Deposit - PBGC Form 710 — If you are already receiving pension benefit checks from PBGC, use this form to have your benefit payment deposited directly into your account with a financial institution that you designate. Use this form also if you need to make any changes in your existing electronic deposit information, including a change in account, account number, routing number, or financial institution name.
  • Election to Withhold Federal Income Tax from Periodic Payments - PBGC Form 719 — If you are already receiving pension benefits from PBGC, either by check or direct deposit, use this form to begin or modify your Federal Tax Withholdings.
  • Payee Information Form - PBGC Form 701 — If you were notified that PBGC is handling your pension plan and you were receiving a monthly benefit when your pension plan ended, please use this form to provide PBGC with basic information about your participation in your plan. Do not use this form if you were not receiving benefits when your pension plan ended.
  • Monthly Payment Election Form - PBGC Form 717 — If you are currently receiving an annual pension payment from PBGC and would like to choose a monthly benefit instead, use this form to elect the monthly payment option.
