My staff can arrange the
purchase of a United States flag that will be flown over the Capitol building
to commemorate a special event or occasion such as an anniversary, wedding, or
birthday. In addition to the flag, you will receive a certificate from the Architect
of the Capitol stating the name of the person for whom the flag was flown, the
occasion for which it was flown, and the date it was flown.

or contact my office in Los Angeles or Washington
to ask for a flag request form. Send the completed form, with your
check made payable to 'Henry Waxman Stationery Account',
to my Washington DC office.
Please allow
at least six weeks from the time that you make your request for
the flag to arrive to you.
The prices below
include the price of the flag, the cost of flying the flag, and
postage.

Terrorist Victims Flag Memorial Program
Through the Terrorist Victims
Flag Memorial Program, we can provide, at no cost to the recipient, a U.S. Flag
flown over the Capitol Building to a surviving victim or the family of a deceased
victim of the September 11, 2001 terrorist attacks. Please download
the Terrorist Victims Flag Memorial Request Form
or contact my office in Los Angeles or Washington to ask for a request form.
Send the completed form to
my Washington DC office.
(Flags may be requested through the Terrorist Victims Memorial Program
by an immediate survivor of a loved one who lost his or her life
in or due to the terrorist attacks of September 11 and subsequently
related rescue and recovery operations or by a surviving victim
who sustained physical injury as a result of the terrorist attacks
and subsequently related rescue and recovery operations.)
