Adding Pages to Your Passport

A passport, except for direct return to the United States, will not be issued in any case in which the applicant has been certified by the Secretary of Health and Human Services as notified by a state agency under 42 U.S.C. 652(k) to be in arrears of child support in an amount exceeding $5,000.00.
Your passport will be revoked if you fall under this regulation, including when you apply for
additional pages, seek a renewal or process a Report of Birth application.
Additional Pages - By Mail
If your full-validity passport has not yet expired, but has no blank pages left, we can
add additional pages for you free of charge by mail. Processing time is only a few days. We can usually only add two sets of pages; after that, you will need to replace the passport even if it has not already expired.
We cannot add additional pages to a limited validity passport. Limited passports have a
duration of less than 10 years (for adults) or less than 5 years (for children).
If you run out of blank pages in a limited validity passport, you must apply for a new,
full validity passport. Details on how to apply are

Application Form for More Pages
Please complete an application form
(DSP-19). Note that the online version of this form includes instructions on the back
side written for use in the U.S. which do not apply to your application in Japan.
This form must be completed, and signed by the passport holder him/herself, regardless of
whether you seek additional pages by mail or in person.
You will need to use free Acrobat software to view and print this form. You can
download the
software.
There is no fee for this service.
Please send us your passport, the completed, required form
DSP-19, a note requesting
additional pages, and a self-addressed, Y270 stamped envelope. Please mail your materials to
the
office serving your part of Japan.
Military members with APO addresses on Okinawa should send their materials to the Consulate in
Naha; all other APO users please send your materials to the Embassy in Tokyo. Our addresses are
here.
Two-Parent Consent Required for Adding Pages
The two-parent passport consent requirement for minors under age 14 applies to all passport
services including requests for extra passport supplement pages.
To comply with this two-parent passport consent statute, you must:
Submit the other
parent's written consent to the application using
this form. You must also include the child's original birth certificate as evidence of the
two parents' names and bring in with you the other parent's original passport.
One parent may sign in person and the other parent give his/her consent through a written
affidavit (One parent signs and submits second parent's signed consent; download the
affidavit form. The form
must be notarized). Follow this link to learn more about these
requirements. Note the parent who signs in
person will need to still have the non-present parent's passport with him/her.
Japanese notarials are acceptable. Notarials done on US military bases are
acceptable. Notarials for this purpose can be done at no charge at the Embassy or one of our
Consulates. All notarized statements must be in English and originals.
If only one parent signs the application with no consent from the other parent, the signing parent must establish sole authority to execute the application through written proof of sole custody, for example, following a divorce.
Or, both parents may appear in person at the Embassy and sign the request for extra passport pages for children under age 14.You must also bring the child's birth certificate as evidence of the two parents' names.
Changing Your Name in Your Passport
Your passport may be amended to show your married name or a legal change of name. Processing
time is only a few days.

You'll need to show us legal documentation supporting any requested change.For example, your original marriage certificate showing your new spouse's name or an original court order showing a legal name change.We cannot accept copies, and we reserve the right to request additional documentation.

Extending a Limited Validity Passport
Limited Validity passports can be issued for a variety of reasons, but usually are designed to cover emergency travel. Such passports, valid for a maximum of twelve months, can only be replaced with a full validity passport.You will need to follow the requirements as explained here and apply in person.
We cannot issue a second limited validity passport to replace an earlier one; you are expected to provide the information needed to enable us to issue a full validity passport following the issuance of a limited one.
There is no additional fee when trading in a limited validity passport for a full-validity passport if you provide the old, limited passport to us for cancellation.Otherwise the fees are $85 for adults and $70 for children.
Since there are various reasons for limiting a passport, a case by case examination must be made by the passport section of the Embassy before determining whether a passport can be extended. Often the passport can be extended only after authorization is received from the Department of State in Washington, D.C. Full validity passports are printed only in the U.S.
Because of these potential delays, you should not make any unchangeable travel plans until we are able to hand you your new passport.
